Please find RVMR form on the CDC website CDC Rabies Vaccination & Microchip Record (RVMR) Please upload your dog(s) CDC Rabies Vaccination and Microchip Record (RVMR) here. An RVMR is required to enter the United States - traditional Rabies Certificates, pet passports, or any other certificates are not accepted. The certificate(s) must be in English. If you have multiple dogs that require reservations for importation, each dog much have an individual RVMR uploaded here.

Additional Veterinary & Vaccine Records All boarders at Rue's Kennels are required to be up-to-date on Rabies, Distemper, and Bordetella vaccines, negative fecal parasite testing, as well as endo- and ecto-parasite prevention. Please provide all of your pet(s) vaccination information and recent veterinary records HERE for our review. We will provide you with an estimate for any additional veterinary care required upon arrival for boarding at our facility.
